Legal Disclaimer & Client Notification

Cotton Phillips Estate Services is not a law firm, and I am not an attorney. The services I provide — including assistance with filings, paperwork preparation, negotiations, guidance through administrative processes, and general organization of your estate or benefit-related matters — are only meant to help you understand your options and navigate available procedures.

Nothing on this site, in any document I provide, or in any conversation with me should be construed as legal advice, financial advisory, or a substitute for the services of a licensed attorney or licensed financial advisor.

If you have legal, tax, or financial issues requiring licensed counsel (for example, drafting legally binding wills/trusts, resolving disputes, interpreting statutes or complex benefit law), I strongly recommend that you consult a licensed professional.

By working with Cotton Phillips Estate Services, you acknowledge that all final decisions about your estate planning, benefit filings, negotiations, or legal arrangements are your responsibility and should be reviewed with an appropriate licensed professional. I cannot guarantee results, compliance, or outcomes.

Client Rights & What You Should Know

  • You have the right to obtain full written copies of any forms, filings, or documents submitted on your behalf.

  • You have the right to review any information I rely on or gather for your case (financial data, benefit eligibility, correspondence, applications).

  • You have the right to choose your own attorney or advisor at any time — even if I helped you get started.

  • You have the right to ask for clarity: what services I’m providing, and what I am not doing (e.g., representing you in court, giving formal legal opinions, giving binding financial advice).

  • You have the right to contact the original benefit or government agency directly (for example, if you believe an error was made).

I commit to transparency and to providing you — as the client — with full access to the resources and information you need to make informed decisions.

Regional / Local Office Contacts (for Midland / Lubbock / Amarillo areas)

While HHSC handles Medicaid statewide, here are some regional health services offices that may be useful depending on your location and needs (especially for clients in West Texas).

Texas Department of State Health Services (DSHS) — Region 1, Lubbock Office

Address: 6302 Iola Ave., Lubbock, TX 79424 — main phone for the region: 806-744-3577

Official Website

DSHS — Amarillo / Region 1

DSHS covers Amarillo under Region 1 (regional provider contact lists show an Amarillo contact: e.g. “Frannie Nuttall, Amarillo” for certain public-health services)

Phone and Email Directory

For general in-person public-health / community-health / benefit-support in Midland

The local county-level or city-level health services may help: e.g. the county or city Health / Senior Services department (if your client is in Midland County). For general state-level Medicaid/benefit inquiries, clients should still start with HHSC or DSHS contacts above.

Note: The statewide portal for benefit application, renewal, and case-status is YourTexasBenefits — all residents in Texas use it for Medicaid, SNAP, TANF, CHIP, etc. The official site is yourtexasbenefits.com
